Point of Care Documentation – Our clinical staff completes documentation in the home setting.  This has proven to improve patient safety, ensure accuracy and timeliness of documentation, and provide staff with patient’s entire medical chart while at the patient’s side.

Anticoagulation Therapy - Anticoagulation medications are used to help prevent blood clots.  Our Registered Nurses can monitor blood levels to ensure patients are receiving therapeutic dosage of medications. 

Medication Reconciliation Every Visit – The process of comparing a patient's medication orders to all of the medications that the patient has been taking. This reconciliation is done to avoid medication errors such as omissions, duplications, dosing errors, or drug interactions.

Disease Specific Education/Interventions – Our clinical staff focuses on treating you and your specific disease.  We provide education to help manage your disease in your home.

Respiratory Therapy – Chronic Respiratory disease management with pulmonary education, rehabilitation and equipment utilization.


Our team at Aquinas Home Health is committed to keeping our patients at home.  We deliver professional care under the direct supervision of your personal physician.  Below are the services we provide:

Patient Specific Plan of Care at Admission – Our clinical staff develops a plan of care to help you meet your goals in the privacy of your own home.  We encourage your input in establishing goals and developing the plan of care. 

Care Transition Program – The goal of this program is to help transition patients from the hospital setting to other care settings, to improve quality of care and reduce readmission into the hospital.

Physical Therapy – Addresses the treatment, healing and prevention of injuries and disabilities. 

Occupational Therapy – Focus on helping patients safely manage their activities of daily living.

Speech Therapy – Includes evaluating and treating patients who have problems with communication, cognition, swallowing and talking.

Certified Oasis Specialist – All home health agencies that participate in the Medicare program are required to collect and send OASIS data to the health care financing administration.  Aquinas Home Health employs a Certified Oasis Specialist to ensure accuracy with this documentation.

Medical Social Worker (MSW) – Assists patients and families in obtaining needed resources and services. 

Wound Vac Certified – Wound Vac promotes healing through Negative Pressure Wound Therapy (NPWT).  All of our Registered Nurses have been trained on the proper use of the Wound Vac.

IV Certified – Since we only utilize Registered Nurses, all of our nurses are trained in the safe administration of IV medications and care of the various types of IV access.

Everyone spent time with me and took care of me like I  was the only patient they had.  It made my wife feel good because I was being  taken care of.  I was very satisfied and would ask for Aquinas again.  I had had home health in the past and didn’t realize the other agency was just fair until Aquinas came.  Aquinas was so good and so thoughtful.
